DOCUMENTS REQUIRED WITH THE FILLED UP REGISTRATION FORM

1. Passport size photograph duly attested.

2. Date of Birth Certificate. ( In case of defence personnel - valid from Part-II Order only) issued from Municipal office.

3. SC/ST certificate (if applicable) from Magistrate only in the name of parent or child) certificate obtained from office will not be considered.

4. Residence proof from office , if not applicable, a copy of Ration Card.

5. Service Certificate.

6. Transfer certificate in the given format only as given on the registration form.

DOCUMENTS AT THE TIME OF ADMISSION

1. Originals of all the documents submitted along with registration form.

2. Affidavit in stamped paper (Rs.2/-) duly attested by notary / first class  magistrate.
